World Patient Safety Day 2026

In a blog to mark World Patient Safety Day, Patient Safety Learning explores what this year’s theme, “Safe care for noncommunicable diseases,” means for patient safety and highlights the work Patient Safety Learning has been doing to support the campaign. This includes a downloadable poster and a series of related blogs on the hub, offering practical insights and resources to help drive action for safer care:

  • Women’s heart health is a patient safety issue
  • The cost of a missed dose: how a new dashboard is transforming getting Parkinson’s medication on time
  • Why infection prevention and control is critical for patients living with noncommunicable diseases
  • From data to safer care: the role of stroke audit in improving quality of care

Recommendations for a future AI regulatory framework

The 10 Year Health Plan for England sets an ambition to “make the NHS the most AI-enabled care system in the world”. In line with this aim, there has been a significant increase in use of ambient voice technology (AVT) products, sometimes referred to as AI scribes, by healthcare providers. Responding to these developments, the Health Services Safety Investigations Body (HSSIB) has recently launched a new investigation exploring the use of AVT in hospitals. In a new blog, Patient Safety Learning considers:

  • Benefits and risks associated with the use of AVT products in healthcare.
  • The evolving regulatory environment around this technology in the NHS
  • Key issues for a new patient safety investigation to consider. 

Transformative Simulation resources

Transformative Simulation considers how simulation can be intentionally used beyond education to understand and contribute to change within healthcare systems. hub topic lead Sharon Weldon shares a number of resources demonstrating how simulation has been used to support patient safety and system improvement in practice.

Black Maternal Mental Health 

This week is Black Maternal Mental Health Week UK. Sandra Igwe, the Founder and CEO of The Motherhood Group, and hub topic lead, introduces this year’s theme: Joy, Justice, Journey, and explains why action must extend beyond the awareness week and why Black maternal mental health is a patient safety issue. She also talks about a new digital maternal wellbeing and community platform. 

Just Culture at Mersey Care

In a new blog on the hub, Beatrice Fraenkel shares her reflections on the approach taken to establish a just culture at Mersey Care NHS Foundation Trust during her 14 years as Chair of the organisation. She explores the relationship between culture, leadership, and patient safety, and how one organisation’s journey shows both the power of getting it right and the risks when it begins to unravel.
